For all characters, picking up a Prototype Xbox item in survival mode will automatically unlock a new costume for them.
Some costumes have multiple ways to be unlocked. If you see a costume with multiple clear conditions, it does not mean you have to clear all of them, but rather that you have more than one way to unlock it.
Note that all costumes except Kasumi's C3 and Ein's C3 cannot be unlocked in the US version of DOA3. You will instead need the Booster Disc or a copy of DOA2 Ultimate to add costumes to your game.
Costume 3:
Complete Kasumi's Sparring Exercise.
Costume 4:
Clear Story Mode without using a continue
Clear Story Mode on Very Hard difficulty
Costume 5:
Clear Tag Battle with Attack, Throw and Hold hitting average 30% or more and total hitting average 70% or more
Costume 3:
Get 20+ wins in Tag Survival with Hayate
Costume 3:
Get 20+ wins in Survival
Costume 4:
Get 20+ wins in Survival again
Get 20+ wins in Tag Survival with Zack
Costume 3:
Get 20+ wins in Survival
Costume 4:
Win a Bronze Award or better in Tag Time Attack with Hitomi
Costume 3:
Win a Gold Award in Time Attack
Win a Bronze Award or better in Tag Time Attack with Brad Wong

Get 20+ wins in Survival
